Eligibility

Serves all ages. MUST:. live in the United States. be at or below 250% of federal poverty level. have an eyeglass prescription dated within the last 24 months. have no other resources available to pay for glasses, including federal/state programs or assistance from local charitable organizations. have not received a New Eyes voucher within the past 12 months

Application must be submitted online by a social worker, a case worker, or an agency.
