This program provides Case Managers that are trained to match seniors' needs in areas such as in-home care, assistance for family caregivers, SNAP, Medicaid, housing, Social Security, Medicare and many forms of assistance that can make seniors and their families' lives easier. One of our Case Managers will be happy to visit with your about your special needs and the programs and services that will be most beneficial to you and your family.

Program provides assistance with reopening clients coverage. If community members lost coverage because they failed to provide requested information, they still can provide that information and have their coverage re-opened. They have 30 days to do so after their closing date if they qualified for Medicaid under Long-Term Services and Supports eligibility rules. They have 90 days after their closing date if they qualified for Medicaid under Modified Adjusted Gross Income (MAGI) eligibility rules.If one loses their card, they should call 1 (800) 482-8988 as soon as they notice their card is missing.NOTE:DHS is beginning the renewal process for ALL Medicaid beneficiaries NOW! If community members receive any request from DHS in the mail, THEY MUST RESPOND WITHIN 30 DAYS. The Northwest Arkansas Food Bank's Mobile Pantry Program makes 17 stops throughout Northwest Arkansas once per month. Food is available to everyone in need. Premade boxes full of shelf stable products, frozen protein and fresh produce is distributed on a first come first served basis.

Program provides information and assistance to clients who contact. Program will connect an Information and Assistance Specialist. The specialist will discuss the situation with the client right then over the phone and if needed will make an appointment for clients with a personal Care Coordinator. The specialist will schedule a time to assess the clients needs and determine the programs and services that would best fit.The personal Care Coordinator will work with the client throughout the process and may refer to the client to other resources and services as needed.
