Change one's PIN if someone knows what one's is or if one has forgotten a PIN. Balance inquiry. Obtain a list of last 10 transactions. Report if a store deducted the wrong amount. Ask a question about using EBT card. In the case of a disaster, the local DHS office will provide food. When help seekers mention the loss of food due to power outages or local disaster, they may request emergency food assistance. For power outage, the form requires proof of time power was lost and when it was restored. Customers have ten (10) days from the time power is restored to submit the request for assistance.
